Chief Complaint:

The patient is a 72 year old woman with a chief complaint of dyspnea on exertion and intermittent chest tightness for five days.

History of Present Illness:

The patient was in her usual state of health until four months prior to admission when she was diagnosed with myelodysplastic syndrome. The patient underwent four cycles of azacitidine for myelodysplastic syndrome, with the last cycle one day prior
